What Cyclospora is — and why it's unlike most foodborne pathogens

Cyclospora cayetanensis is a microscopic parasite that causes an intestinal illness called cyclosporiasis. It was first identified and named in the early 1990s and is exclusively a human parasite — unlike Salmonella or E. coli, no animal reservoir exists. Cattle, poultry, pigs, and wildlife do not carry the species that causes human illness.

The illness itself is not subtle — the trademark symptom is prolonged, watery diarrhea that can persist for weeks without treatment and, for older adults, young children, and immunocompromised individuals, can require hospitalization. Fatality rates are low, and an effective antibiotic treatment known as trimethoprim-sulfamethoxazole exists.

The limits of detection for Cyclospora

Cyclospora cannot be grown in a laboratory, which means detection relies on molecular methods that identify DNA sequences unique to the parasite. Those methods identify the parasite's presence but cannot determine whether organisms are alive and capable of causing infection. Once produce is contaminated, no testing protocol reliably eliminates the risk.

The incubation period — typically a week, sometimes longer — gives contaminated food time to move far from its source before anyone notices a pattern. By the time patients are interviewed, they struggle to recall what they ate days earlier, especially stealth ingredients like cilantro or lettuce that rarely register as distinct meal components.

Produce supply chains involve multiple growers, processors, distributors, and retailers, often without standardized traceability records — which means investigators trying to work backward from a sick patient to a contaminated field are reconstructing a chain of custody that was never fully documented in the first place.

Even when a food source is suspected, molecular characterization tools for Cyclospora remain far less discriminatory than whole-genome sequencing for bacteria, making it difficult to confirm whether cases in different states share a common source or represent separate events entirely.

What the 2026 Cyclospora case numbers reveal about underreporting

At the time of recording, CDC was reporting approximately 7,000 lab-confirmed domestically acquired cases, with more than 11,500 additional cases pending analysis — bringing total reported illnesses to roughly 18,000 across 45 states.

According to a 2011 CDC study cited by Frank, roughly 83 Cyclospora infections occur for every confirmed case — a multiplier Frank acknowledges has likely fallen with improved awareness and testing, but one that puts the true 2026 toll potentially past one million cases.

The burden of illness pyramid illustrates why — most people who get sick never show up in the data at all, either because they didn't seek care, weren't tested for the right pathogen, or recovered before anyone connected their illness to the correct food source.

How changes to FoodNet affected the speed of the response

FoodNet — the joint CDC-FDA-USDA active surveillance system — shifted Cyclospora to optional reporting in July 2025. The CDC's weekly Cyclospora case summaries were also paused, limiting public visibility into how quickly cases were rising and where.

In Frank’s view, these changes affected the ability to detect the signal early, assess its magnitude quickly, and coordinate a timely response.

In outbreak response, surveillance is the early warning system. When it goes passive on a pathogen already this difficult to track, the signal arrives late — and in a Cyclospora season, late means more people sick before anyone can act.

How the traceability rule delay slowed the 2026 response

The FDA's Food Traceability Rule — requiring standardized digital recordkeeping for high-risk foods including leafy greens — was delayed from January 2026 to July 2028. Frank says conversations with investigators working multiple clusters suggest that delay slowed traceback considerably.

“Asking investigators to conduct traceback without modern traceability tools is like asking an emergency responder to get to an accident scene quickly without GPS.” — Frank Yiannas

Faster access to standardized records — which farm, field, harvest dates, and distribution channels received the product — would have allowed investigators to answer critical questions sooner, narrow advisories rather than cast them broadly, and conduct true root cause investigations.

What the 2020 outbreak proved about agricultural water

Frank points to the 2020 bagged salad outbreak — linked to a product containing iceberg lettuce, red cabbage, and carrots — as one of the most important Cyclospora investigations on record. Traceback pointed to a farm in South Florida, and FDA environmental sampling detected Cyclospora in a regional water management canal supplying irrigation water to the area.

Agricultural water is a critical control point — and in Frank's view, one of the most important areas for future prevention focus.

What prevention requires — and what research still needs to solve

According to Frank, prevention requires movement on five fronts: full implementation of the FSMA Produce Safety Rule on the ground; improved farmworker hygiene infrastructure and training; rigorous agricultural water quality management; reducing Cyclospora infections in farming communities at the source; and industry-led action that goes beyond regulatory compliance.

“I do not believe we should accept Cyclospora as a seasonal nuisance. I believe that with leadership, commitment, science, and action, contamination of produce with Cyclospora can be reduced, and in many circumstances prevented.” — Frank Yiannas

Looking ahead, three research advances would change what investigators can do. Better genetic characterization tools would bring the same source-linking precision to Cyclospora that whole-genome sequencing brought to bacterial pathogens. Methods to inactivate oocysts in agricultural water would close one of the most significant contamination pathways identified to date. And culturing Cyclospora in a laboratory — something no one has yet achieved — would open lines of inquiry that are currently closed off entirely.

The field already has a regulatory framework, a documented contamination pathway, and a clear research agenda. What Frank argues the industry has not yet demonstrated is the commitment to treat Cyclospora as a solvable problem rather than an annual inevitability.
